    Education and Support, Survivor-to-patient/family visitation, Tangible Services  (medical equipment, comfort pillows, blue pads, ostomy supplies and other materials related to rehabilitation.  The Guestroom Program provides temporary housing for cancer patients who must travel for outpatient treatment and checkups.  Volunteers provide transportation to and from cancer-related treatment.     AWARE offers free AIDS education and testing, counseling and referrals for substance abuse, treatment, early intervention for people who are HIV positive or diagnosed with AIDS.  They also do TB testing. They have a food pantry, clothing closet, emergency assistance for utilities, and prevention case management for high-risk individuals.

    It supports the needs women and families in the greater Canyon area, which includes Happy and Hereford. The center offers pregnancy testing, peer counseling, parenting classes, childbirth education classes and 24 hour helpline. These services are free.

    Can assist families in getting their children signed up for CHIPS insurance.  This insurance is for families that make too much to qualify for Medicaid.  It can also assist families in determining which program they qualify for and assist pregnant women with CHIP prenatal to cover prenatal appointments and delivery. This is also for women who don't qualify for medicaid because of their income or immigration status.

    This is a place that provides health care and prescriptions. It's a "last resort" meaning that families should apply for Medicaid and CHIPS first to see if they qualify. If they do not qualify for those programs, they can apply here for medical assistance and pay on a sliding scale. They also take Medicaid, CHIPS, other insurance. If a person does not have a Social Security number, they can get help here.

    This a program through United Way Helpline that helps families with needed medication.  They look up the medication in the MAPS folder and give the family an 800 number to call.  If they qualify, they can get the medicine free.

    They assist patients who may qualify to enroll in one or more of the many patient assistance programs now available. These programs provide prescription medicine free-of-charge to individuals in need, regardless of age, if they meet the sponsor's criteria.

    If a child is on Medicaid (ages 0-21), they are eligible for "Medical Case Management".   The case manager helps with family problems, school/education issues, transportation problems, identifying services and making referrals in your community, getting and keeping appointments, attendant care and overcoming barriers for families.  Children who received SSI are assigned a case manager. You can call 655-7151 to see who is the assigned case manager.

    The Medical Transportation Program (MTP) helps Medicaid recipients get to Dr., dentist, counseling, hospital and pharmacist appointments. You can call the toll free number Monday-Friday 8am-5pm. There are 3 ways they can help: If you have a car, they will help with gas money, they will pay for someone else to take you or they will arrange a bus or taxi to pick you up and take you. You must give them 24 hours notice.

    Sight for Students is a VSP charity that provides free vision exams and glasses to low-income, uninsured children. The program operates nationally through a network of community partners who identify children in need and VSP network doctors who provide the eye care services.

    This is a place homeless families can go get health services. Eligibility is based on family size and income. They use a sliding scale. Proof of income is required. They also assist families without a social security card.
